Provide community access and home activities and opportunities to engage in the community.
To provide increased social networks opportunities with community, family members or others that may not be intellectually disabled; To provide access to public goods/services; to promote individuals as recognized community members, valued social role and trusted to perform social role in community, belong to social networks within which one receives and contributes support; To provide activities, social roles and relationship with nondisabled citizens; To provide interaction with others and access to community facilities; To provide activities and supports, which best meet each individual's personal preferences and interest.
To create community resources that will provide various opportunities of learning.
To make a difference in helping each individual chose a meaningful lifestyle.
MAJOR DUTIES:
Safely transport individuals to their desired activities, assist with building relationships, implement person centered plans.
Document support logs daily and review all support logs.
Complete quarterly reports for each individual.
Complete and manage monetary ledgers for each individual.
Maintain an environment that implements quality health and safety management standards to protect the health, safety and welfare of individuals.
Maintain confidentiality and security of individual support records Seek out community opportunities that facilitate building relationships ALL OTHER DUTIES AS ASSIGNED QUALIFICATION REQUIREMENTS:
TOVA FIRST AID/ CPR VALID DRIVER's LICENSE MUST HAVE AN EXCELLENT DRIVING RECORD HIGH SCHOOL DIPLOMA or HIGHER Satisfactory Criminal Records Report and Background Check HCBS Home Community Based Services Training) Human Rights STATUS:

Supervising individuals with a wide variety of support needs Written, oral, and interpersonal communication Implementing program supports and with outside agencies, organizations, and professionals Maintaining professional ethical standards Satisfactorily complete all required training in First Aid, CPR, Medication Management, Behavioral Management, etc.
